UNDERSTANDING GROIN PAIN

A crowded crossroad —
so precision matters.

The groin packs muscles, tendons, the hip joint, and abdominal wall into a small space. Pain here falls into distinct types — adductor-related, hip flexor-related, inguinal-related, pubic-related, or hip-joint-related — and each one is treated differently. Getting the entity right is the whole game.

Most groin pain in active people is performance-limiting rather than season-ending: you can still play, but sprinting, cutting, and kicking hurt. That makes it tempting to play through — and easy to let it become a months-long problem.

The target is pain-limited, not pain-free activity. Discomfort up to about 3 out of 10 during exercise that settles afterward is acceptable and part of reloading; sharp pain is not.

Does this sound familiar?

  • Pain when sprinting, changing direction, or kicking
  • Hurts to squeeze your legs together against resistance
  • A deep ache where the leg meets the body
  • Stiffness in the groin after sitting, easing once you move
  • It keeps coming back every season
01

Five types, five treatments

Adductor, hip flexor, inguinal, pubic, or hip-joint — each groin pain type has its own exam findings and its own rehab path. We diagnose first, then treat.

02

Play hurt — within limits

Mild discomfort (up to ~3/10) that settles after activity is part of reloading, not a stop sign. Sharp or worsening pain is.

03

Strength numbers must be absolute

Matching your other leg is not enough for return to sport. We measure actual adductor strength against published targets before clearing you.

⚠ Tell us promptly if you notice:

  • A visible bulge in the groin, especially when coughing or straining
  • Testicular pain or swelling
  • Fever, or groin pain after a major trauma
  • Numbness, or pain that nothing — rest or movement — changes
  • Unexplained weight loss with persistent groin pain

A bulge can mean an inguinal hernia, which needs surgical evaluation — not rehab first. We screen for these at your first visit and refer promptly when the picture does not fit a muscle or joint problem.

HOW PHYSICAL THERAPY HELPS

Diagnose precisely.
Load progressively.

The right rehab for the right structure.

✓Precise entity diagnosis

We use structured testing to identify which of the five groin pain types you have — because adductor rehab will not fix a hip joint problem.

✓Adductor strengthening

Targeted adductor loading — from isometrics through the Copenhagen program — dosed to your irritability, not to a generic protocol.

✓Whole-chain control

Groin pain lives in how the pelvis, trunk, and leg work together. We train intersegmental control so the adductor is not overloaded every stride.

✓Progressive return-to-sport testing

Sprint, cut, and kick progressions with strength targets — you return when the numbers say so, not when the calendar does.

✓Load monitoring

We track your training load against your current capacity so the same error does not recreate the same injury next season.

✓Referral coordination

Suspected sports hernia or hip joint pathology gets prompt referral for imaging and surgical opinion — we do not rehab what needs a surgeon.

GROIN PAIN FAQ

Questions we hear most

Straight answers before you book.

Do I have to stop playing completely?
Usually not. Most groin pain is managed with modified activity — discomfort up to about 3/10 that settles afterward is acceptable. Complete rest often just delays the rebuild.
Is it a sports hernia?
True sports hernias (inguinal-related pain) are one of five groin pain types — less common than adductor problems. Our exam differentiates them, and a visible bulge means prompt surgical evaluation.
How long until I can play fully again?
Adductor-related pain often improves over 8–12 weeks of progressive loading; return depends on hitting strength and performance targets, not just feeling better.
Will an MRI show what is wrong?
Imaging often shows findings — like pubic bone changes — in athletes with no pain at all. Diagnosis comes from the exam matched to your story; imaging confirms, it does not decide.
Why does it keep coming back every season?
Usually because the original rehab restored comfort but not capacity — adductor strength and load management were never rebuilt. We fix the underlying deficit, not just the pain.
