UNDERSTANDING NECK PAIN

Your neck is strong.
It may just be undertrained.

Most neck pain is mechanical — sensitive muscles, joints, and sometimes a nerve — rather than damage. It flares with sustained positions, stress, poor sleep, and workload, and it settles as the neck gets stronger and moves more.

When a nerve root in the neck is irritated (cervical radiculopathy), pain, tingling, or numbness can travel into the shoulder, arm, or hand. That pattern needs a proper neurological check — strength, reflexes, sensation — which is part of every neck evaluation here.

Here is what research keeps showing: the deep muscles that support the neck switch off with pain and do not fully come back on their own. Specific training for those muscles, progressed over weeks, is one of the best-supported treatments we have — and it beats rest, heat, and hoping.

Does this sound familiar?

  • Aching or stiffness in the neck and shoulders
  • Pain between the shoulder blades
  • Headaches that seem to start from the neck
  • Pain, tingling, or numbness running into the arm or hand
  • Pain when turning your head — driving, checking a blind spot
  • Neck pain that builds over the workday at a desk
01

Your neck is built to move

The neck carries your head all day, every day. Guarding it and holding it still usually makes it stiffer and more sensitive — graded movement is the way out.

02

Posture is about variety

There is no single perfect posture. The problem is usually staying in any one position for hours. The best posture is your next posture — small, frequent changes beat expensive chairs.

03

Scans show age, not destiny

Disc bulges and "wear" appear on MRI in most adults with zero pain. A scan finding alone does not tell us why you hurt or what to do about it.

⚠ Get medical care promptly if you notice:

  • Arm or hand weakness that is progressing, or new clumsiness — dropping objects, trouble with buttons
  • Balance problems or changes in how you walk
  • Neck pain after a fall or accident, or with fever
  • Unexplained weight loss, or severe night pain that never eases

These are uncommon, but they need medical assessment first — we will tell you directly if that applies to you.

HOW PHYSICAL THERAPY HELPS

Train the neck you have,
not the one in a textbook.

Your program is matched to your pattern — local neck pain, nerve symptoms, or both.

✓A real evaluation first

History, movement testing, and — whenever arm symptoms are present — a full neurological screen: strength, reflexes, and sensation. We also ask about sleep, stress, and workload, because they drive neck pain more than most people realize.

✓Deep neck muscle training

Gentle, precise exercises wake up the deep supporting muscles of the neck that pain switches off. This is specific, skilled training — not generic stretching — and it is progressed as your control improves.

✓Progressive neck & shoulder strengthening

As symptoms settle, we build strength in the neck, shoulder blades, and upper back so your neck tolerates work, training, and long days again.

✓For arm & nerve symptoms

Positions and movements that ease arm pain come first. Gentle nerve mobility work and graded loading follow, with your arm strength re-checked along the way so progress — or any problem — is caught early.

✓Hands-on care when it helps

Manual therapy can reduce pain and stiffness in the short term and make exercise easier. We use it to support your active program, never instead of it.

✓Desk & daily-life strategy

Simple changes to how you set up, move, and take breaks during the day — built around your actual job, not a generic ergonomic checklist.

NECK PAIN FAQ

Questions we hear most

Straight answers before you book.

Is my posture — or my phone — causing this?
Posture alone rarely causes neck pain. What matters more is how long you stay in one position and how strong and resilient your neck is. We would rather make your neck stronger than make you afraid of your phone.
Do I need an MRI of my neck?
Usually not to start. Most neck pain — including many cases with arm symptoms — is evaluated well by history and physical exam. Imaging makes sense with red flags, progressive weakness, or no progress over time, and we will tell you if that is you.
My arm tingles into my hand. Is that serious?
It usually means a nerve in the neck is irritated. Most cases improve with guided conservative care, but it does need a proper neurological exam and monitoring — which is built into our evaluation and follow-ups.
How long does neck pain take to settle?
Many people feel real change within a few weeks of a specific training program. Longer-standing pain tends to improve over weeks to a few months as strength and control return. We will give you an honest estimate at your evaluation.
Should I get a special pillow or collar?
A soft collar usually weakens the neck if worn for long, and no pillow fixes an undertrained neck. Comfortable sleep matters, but the treatment is training — we will help you find sleep positions that calm symptoms while you build strength.
Do I need a referral to start?
In most cases, no. New York allows direct access to physical therapy. Some insurance plans still require a referral — we will confirm when we verify your benefits.
